What are PESA and TESA?
PESA (Percutaneous Epididymal Sperm Aspiration) and TESA (Testicular Sperm Aspiration) are minor
surgical procedures used to collect sperm directly from the male reproductive tract when none are
present in the ejaculate.
- PESA involves extracting sperm from the epididymis (the tube where sperm mature).
- TESA involves extracting sperm directly from the testicular tissue.

Q: What is the difference between PESA, TESA, and TESE?
A: PESA and TESA are needle-aspiration techniques. TESE (Testicular Sperm Extraction) is a more invasive open biopsy that removes a small piece of testicular tissue. We start with the least invasive option (PESA/TESA) first.
Q: Will PESA/TESA affect my testosterone levels or masculinity?
A: No. These procedures are designed to retrieve sperm without affecting the testicle’s primary function of producing testosterone. Your hormone levels and male characteristics will not be altered.
Q: If sperm is found, can it be used for a normal IVF cycle?
A: The number of sperm retrieved is usually very low and not motile. Therefore, they are always used with ICSI, where a single sperm is injected directly into an egg.
